Care Coordinator – Homecare – Tower Hamlets
Salary: Up to £30,000 (depending on experience)
Working hours: 8:30 am – 5:00 pm, Monday to Friday, plus on‑call as per the rota every 6 or 7 weeks.
Main responsibilities as Homecare Care Coordinator
- Proactively manage new care package referrals.
- Allocate Homecare care workers' rota and workloads on the electronic management system.
- Match care workers to service users based on care need, skills, experience, and personalities.
- Maintain effective links and communication with local authority teams, care workers, service users, relatives and other stakeholders.
- Follow up on any concerns raised by care workers and liaise/referral to appropriate professionals, including GPs, OTs and DNs.
- Ensure care workers are appropriately trained and able to carry out their duties and responsibilities effectively and sensitively.
- Maintain efficient administrative systems and records.
- Implement and monitor all care packages and care plans as detailed within individual service agreements.
- Record and report any complaints whilst ensuring that all quality standards, complaints and comments are dealt with promptly.
- Deal with emergencies arising from the service.
- Manage the homecare service and staff in accordance with the organisation’s aims and objectives, policies and procedures.
- Assist in reviews of all Homecare care worker inductions and training needs on a regular basis to meet the annual training requirements.
- Monitor and record absence for allocated staff.
- Carry out regular supervision and monitoring of Homecare care workers in line with the service’s registration requirements.
- Provide an emergency “on‑call” service as per rota.
- Undertake quality assurance processes, including telephone monitoring calls and care record audits.
About you
- Previous experience working as a Care Coordinator is desirable, or considerable experience within Home Care.
- Understanding of CQC regulations and requirements.
- Excellent IT skills and competency in Microsoft Office packages as well as databases or people rostering systems.
- Excellent verbal and written communication skills with an eye for detail ensuring accuracy of data.
- Ability to work under pressure with changing demands that require responsiveness and flexibility.
- Excellent people skills with the ability to provide an exceptional level of service.
- Experience of prioritising tasks and meeting deadlines without compromising accuracy.
