The predicted salary is between 18000 - 42000 £ per year.
· Meeting service in their own homes as the initial point of contact and fully assessing people’s care needs and individual requirements.
· Completing thorough care assessments and compiling a detailed person-centred care plan to support customers to achieve their desired goals and outcomes, using our real time electronic care planning system.
· Reviewing care plans regularly and updating when there is an identified change in need.
· Assessing risks and compiling risk management plans.
· Follow up on any reported changes to customer’s condition or care needs, including making referrals to appropriate health care professionals.
· Work in partnership with customers, their families, carers, advocates and other health and social care professionals to ensure individual and personalised care.
· Work collaboratively with our officed based Care Coordinators to support the planning of realistic care worker rotas, matching needs, skills and personalities.
· Conducting supervisions, spot checks, observations of care and coaching to care workers in the community.
· Shadow new care workers in the community to ensure confidence and competence.
· Provide support and advice to care workers, including in emergencies or if there is a reported challenge.
· Promote safe working practices by following company policies and procedures.
· Promote understanding of legislative and regulatory requirements.
· Report as necessary on any accident/incident or safeguarding concerns raised.
With previous experience as a Community Care Coordinator, Field Care Supervisor or extensive experience as a Home Care worker ready to make the next step you will be passionate about providing the highest quality care to our service users. The majority of your time will be spent working with the community – visiting service users in their own homes, as well having dedicated time within the office to complete necessary documentation., · Previous experience as a Community Care Coordinator or Field Care Supervisor within a Homecare setting, or a care worker who has considerable experience looking to further develop.
· Understanding of CQC regulations and requirements.
· Excellent communication and written skills.
· Strong IT skills.
· Level 3 (or equivalent) qualification in Health and Social Care, or a Level 2, with desire for progression to Level 3.
· A Team player – able to support the care worker team and show genuine understanding and empathy to lone workers.
· Understanding of the Care Quality Commission (CQC) regulations and requirements.
· Passionate about uncompromised delivery of high-quality care
· Be flexible to the needs of the business as required and be involved in the delivery of care where there are staff shortages.
· A warm and caring team player with a real passion for working as part of the team to ensure delivery of high-quality care.
· Willingness to learn, attend further training programmes, such as train the trainer courses, to support you and to support our care workers in the community.
· Salary: Up to £30k (depending on experience)
· Flexible working Hours – plus on call as per the rota every 6 or 7 weeks.
