About the role: To provide high-quality, person-centred care and support to adults living in rural communities across Stirlingshire, enabling them to maintain independence, dignity, and quality of life within their own homes.
Key Responsibilities:
- Deliver care and support services as outlined in individual care plans, including personal care, medication administration, meal preparation, and companionship.
- Support individuals with mobility, physical disabilities, and long-term health conditions.
- Promote independence and assist with tasks such as shopping, housework, and attending community activities.
- Travel independently between rural service users' homes, often in areas with limited public transport.
- Establish and maintain professional relationships with service users and their families, respecting their privacy and choices.
- Monitor and report any changes in health or wellbeing to the Care Coordinator or relevant health professionals.
- Maintain accurate and timely care records using digital or paper-based systems.
- Adhere to Health and Social Care Standards and company policies, including safeguarding, confidentiality, and health and safety.
