Exam Works UK is recruiting a Social Prescriber Link Worker to support health and wellbeing by connecting people to community services.
Hybrid role based in Northamptonshire, 26 hours per week, permanent contract, with a salary of £27,000–£30,000 per annum (pro rata).
You will work in a multi-disciplinary team, co-produce personalised support plans, and build strong links with local voluntary and statutory organisations to improve outcomes.
