About the Role
As a Social Prescribing Link Worker, you’ll empower individuals to take control of their health and wellbeing by connecting them to non-medical community resources. Working alongside GPs, Advanced Nurse Practitioners, and First Contact Physiotherapists, you’ll take a holistic approach to supporting people with complex social needs.
Key Responsibilities
- Manage a caseload of individuals referred from primary care
- Conduct comprehensive assessments and develop personalised care plans
- Identify barriers to wellbeing and connect people to relevant community services
- Use coaching techniques to promote self-management and behaviour change
- Build relationships with local groups and help develop new community initiatives
Skills & Experience
Essential:
- NVQ Level 3 in Social Prescribing (or willingness to work towards)
- Experience in health, social care, or community development
- Strong communication, coaching, and rapport-building skills
- Ability to work independently and collaboratively within a multidisciplinary team
- Understanding of social determinants of health and long-term condition management
Desirable:
- Training in health coaching, motivational interviewing, or behaviour change techniques
