Exam Works UK is seeking a Social Prescriber Link Worker to join our Health & Wellbeing team in Northamptonshire.

This permanent, part-time position is hybrid, 26 hours per week, with a pro rata salary of £27,000–£30,000.

You will work one-to-one with individuals referred by GPs, co-produce personalised support plans based on goals, connect people to community groups and voluntary services, and build strong, non-judgemental relationships.
