Health and Wellbeing Team Leader
Location: Hybrid (Remote and in-person across Bedfordshire/Buckinghamshire) Contract: Full Time, Monday to Friday Salary: £32,000–£36,000 pro rota and depending on experience.

About the Role
We are seeking an experienced Health & Wellbeing Coach Team Leader to lead a team of Health & Wellbeing Coaches within Primary Care. The role combines leadership and clinical responsibilities, including team management, KPI oversight, service development, and maintaining a personal caseload supporting patients with persistent pain and long‑term conditions.
Key Responsibilities
- Lead, manage, and support a team of Health & Wellbeing Coaches.
- Deliver regular 1:1 supervision, appraisals, and development reviews.
- Monitor performance against KPIs and service targets.
- Identify training and development needs.
- Support recruitment, induction, and onboarding.
- Promote a positive, high‑performing team culture.
Service Delivery
- Monitor patient activity, waiting times, outcomes, and data quality.
- Analyse performance and implement improvement plans.
- Build effective relationships with PCN stakeholders and multidisciplinary teams.
Clinical Responsibilities
- Manage a personal caseload delivering health and wellbeing coaching.
- Support patients with persistent pain and long‑term conditions.
- Facilitate lifestyle and behaviour change goals.
- Use coaching techniques to improve confidence, self‑management, and wellbeing.
- Deliver personalised, patient‑centred care plans.
About You
Essential Experience
- Leadership or line management experience.
- Experience delivering supervision, appraisals, and staff development.
- Experience managing performance and KPIs.
- Experience in health, community, or social care settings.
- Experience working within multidisciplinary teams.
- UK Driving Licence.
Skills & Knowledge
- Strong understanding of behaviour change and self‑management approaches.
- Skilled in coaching, goal setting, and action planning.
- Ability to analyse performance data and drive improvements.
- Excellent communication, organisational, and IT skills.
- Understanding of Primary Care Networks and personalised care.
Qualifications
- Health Coaching qualification (PCI‑accredited or equivalent) or relevant experience.
- Willingness to complete further accredited training if required.
