Social Prescribing Team Leader

About the Role
An exciting opportunity has arisen for an experienced and motivated individual to step into a Social Prescribing Team Leader position within our Pure Health and Wellbeing Social Prescribing service.
This role is key to supporting the ongoing success and development of our service by providing strong leadership, supervision, and operational support to our Social Prescriber Link Worker team. You will play a vital role in promoting high-quality service delivery, strengthening partnerships across Primary Care Networks (PCNs), and embedding a positive, collaborative team culture aligned with our organisational values.
Key Responsibilities
- Provide line management to Social Prescriber Link Workers, including regular supervision, 1-1s, performance support, and appraisals
- Create and maintain a proactive, supportive, and positive team environment
- Support Service Leads with operational duties such as audits, reporting, diary oversight, and KPI monitoring
- Act as a role model for best practice in social prescribing
- Lead on the organisation and delivery of Continuous Professional Development (CPD) sessions
- Support team members with professional development, reflective practice, and career progression
- Build and maintain strong working relationships with PCNs, PCN Managers, and key stakeholders
- Represent the service in partnership meetings, external forums, and community initiatives
- Promote the service through social media, networking, and community engagement (in line with company policy)
- Champion the organisation’s mission, values, and reputation both internally and externally
About You
We are looking for someone who is passionate about social prescribing and ready to take the next step into leadership.
You will have:
- Significant experience as a Social Prescriber Link Worker
- Strong organisational and communication skills
- An empathetic, inclusive leadership style
- Experience of leading, supporting, and supervising staff
- Experience working closely with Primary Care Networks (PCNs) and community partners
- Confidence in using social media and public platforms to promote services
- Play a key role in shaping and developing a growing service
- Work within a collaborative and forward-thinking team environment
- Contribute to meaningful work that improves health and wellbeing outcomes in the community
