About the Role

As a Health and Wellbeing Coach, you’ll be a key member of our Pain Management Team, providing personalised support to individuals referred by clinicians and local agencies. Your work will empower people to set and achieve meaningful health goals, particularly around managing persistent pain and long‑term conditions within the Primary Care Network. This role is ideal for someone who thrives on helping others build confidence, autonomy, and resilience in their health journey.

Key Responsibilities

  • Deliver patient‑centred pain management support and lifestyle coaching.
  • Build confidence and engagement in positive lifestyle changes.
  • Use coaching techniques to help patients set and achieve personal goals.
  • Promote autonomy and self‑management.
  • Support individuals with low activity levels through reflection, planning, and motivational strategies.

Clinical Responsibilities

  • Use health coaching approaches to empower individuals in managing their own care.
  • Provide self‑management education, peer support, and social prescribing.
  • Encourage proactive health behaviours and help prevent new issues from developing.
  • Communicate effectively to support patient choice and positive risk‑taking.
  • Work collaboratively with multidisciplinary teams to guide patients toward meaningful health outcomes.
  • Communicate clearly to enhance patient understanding and rehabilitation.
  • Navigate and overcome communication barriers, including illness beliefs, perception challenges, and cultural differences.
  • Share complex patient information appropriately to ensure coordinated, high‑quality care.

Knowledge and Experience

  • Experience working within a multidisciplinary team.
  • Background in health, social care, or community development.
  • Ability to collaborate effectively with partners across sectors.
  • Understanding of social determinants of health and long‑term condition management.
  • HWC Cert 1 desirable but not essential as training can be provided.
