This ongoing locum position demands an expert practitioner with deep clinical expertise in managing oral and parenteral anticoagulation therapies, monitoring complex patients, and mitigating thrombotic and haemorrhagic risks. Key Duties and Responsibilities * Lead outpatient and inpatient anticoagulation clinics, performing comprehensive clinical reviews for patients prescribed Vitamin K Antagonists (e.g., Warfarin), Direct Oral Anticoagulants (DOACs), and Low Molecular Weight Heparin (LMWH). * Execute Near-Patient Testing (NPT) for International Normalised Ratio (INR) monitoring, interpret lab results, and adjust medication dosages autonomously in accordance with Trust protocols. * Manage complex anticoagulation cases, including peri-operative bridging therapy plans, cancer-associated thrombosis (CAT), deep vein thrombosis (DVT), pulmonary embolism (PE), and mechanical heart valve protocols. * Utilize specialized clinical software systems (such as DAWN Anticoagulation software) to maintain accurate patient dosing histories, audit records, and recall schedules. * Deliver expert educational counselling to patients, carers, and secondary care staff regarding anticoagulation complianc...
