This senior clinical post requires a thorough understanding of hospital pharmacy operations, acute ward-level medicines management, and clinical therapeutic monitoring across complex inpatient directorates.
Core Responsibilities & Clinical Duties:
- Deliver high-quality clinical pharmacy services to allocated hospital wards, undertaking daily medicines reconciliation, clinical chart checks, and discharge planning.
- Provide expert advice on dosage adjustments, therapeutic drug monitoring, and drug interaction management to medical and nursing staff.
- Ensure full compliance with medicines optimisation policies, NHS drug formularies, and clinical governance frameworks.
- Support dispensary operations, aseptic preparation services, and clinical audit initiatives within the pharmacy directorate.
Essential Requirements:
- Full GPhC Registration as a Pharmacist.
- Post-graduate Diploma in General Pharmacy Practice (or equivalent clinical experience).
- Demonstrable NHS hospital pharmacy experience at Band 6 or Band 7 level.
- Independent Prescribing (IP) qualification is highly advantageous.
