Evolve Staffing Ltd. is seeking an experienced Band 7 Clinical Anticoagulation Specialist to take on a key role within a busy acute NHS Trust located in Guildford, Surrey. This ongoing assignment involves leading nurse/pharmacist-managed anticoagulation services, providing expert oversight for outpatient clinics and inpatient referral consultations across acute hospital departments.

Key Duties and Responsibilities

  • Oversee the safe management and administration of therapeutic anticoagulation regimes for patients with deep vein thrombosis (DVT), pulmonary embolism (PE), atrial fibrillation (AF), and valvular heart disease.
  • Perform point-of-care capillary blood sampling and INR testing, authorizing appropriate Warfarin dose adjustments and monitoring intervals.
  • Initiate and review patients commencing Direct Oral Anticoagulants (DOACs) or Low Molecular Weight Heparin (LMWH), evaluating renal function, hepatic profile, and co-medications for safety.
  • Support peri-operative anticoagulation planning and bridging therapy protocols for patients undergoing elective surgeries or invasive diagnostic procedures.
  • Manage specialized digital dosing systems (such as DAWN Anticoagulation software) and participate in clinical governance, audit routines, and thromboprophylaxis risk assessments across the Trust.

Shift Timings and Pattern

The position offers full-time working hours with fixed daily shift timings of 08:30am to 16:30pm, Monday to Friday (37.5 hours per week).

Requirements and Qualifications

  • Professional registration with the Nursing and Midwifery Council (NMC) or General Pharmaceutical Council (GPhC).
  • Extensive experience as a Band 6 or Band 7 Specialist Nurse or Clinical Pharmacist in an NHS anticoagulation or haematology service.
  • Direct experience utilizing DAWN clinical dosing software and near-patient INR testing equipment.
  • Strong communication skills with a proven track record of autonomous clinical decision-making.
  • Full Right to Work in the UK and active NHS compliance documentation.
