Evolve Staffing Ltd. is seeking an adaptable General Practitioner (GP) for an ongoing primary care post based in Gosport, Hampshire. This role offers excellent continuity of care, competitive daily rates, and flexible working arrangements within a well-established primary care setting. The appointed doctor will deliver general medical services, face-to-face consultations, telephone triage, and chronic disease management to a diverse local patient population, supporting the practice and local Primary Care Network (PCN) in delivering outstanding patient outcomes. Core Clinical Responsibilities: * Deliver face-to-face clinical consultations, home visits, and remote telephone or video triage for acute and chronic medical presentations. * Manage complex long-term conditions, chronic disease monitoring, and multi-morbidity care plans in accordance with Quality and Outcomes Framework (QOF) clinical standards. * Prescribe medications safely and appropriately using the practice's electronic clinical system (EMIS Web or SystmOne), adhering to local prescribing formularies. * Process clinical administration efficiently, including electronic laboratory results, hospital discharge summaries, clinical coding, and medical referrals. * Participate in multi-disciplinary safeguarding meetings, palliative care reviews, and clinical practice audits.
