The Opportunity
We’re seeking a dynamic, experienced healthcare professional to join us as an Associate Case Manager. This is a flexible, self-employed role ideal for someone who thrives on autonomy, clinical challenge, and meaningful client relationships.
- Deliver proactive, client-centred case management for individuals with complex injuries
- Coordinate rehabilitation pathways and support return-to-function goals
- Liaise with multidisciplinary teams to ensure seamless care

Based in Middlesbrough, we serve clients across the North East and North Yorkshire.
What You’ll Bring
We’re looking for someone with:
- A relevant professional qualification (Social Worker, Occupational Therapist, Physiotherapist, or Nurse)
- At least 3 years’ post-qualification experience, including work with brain injury, spinal cord injury, and/or amputation
- Minimum 2 years’ experience in case management
- Registration with your professional body
- Strong knowledge of rehabilitation pathways
- Self-employed status with professional indemnity insurance
- A car and business insurance for travel
Pay & Checks
- Competitive self-employed rates (negotiable based on experience)
- Enhanced DBS required (we can arrange this if needed)
- Proof of right to work in the UK, professional registration, references, and driving licence (minimum 12 months driving experience) required
