The Opportunity

We’re seeking a dynamic, experienced healthcare professional to join us as an Associate Case Manager. This is a flexible, self-employed role ideal for someone who thrives on autonomy, clinical challenge, and meaningful client relationships.

  • Deliver proactive, client-centred case management for individuals with complex injuries
  • Coordinate rehabilitation pathways and support return-to-function goals
  • Liaise with multidisciplinary teams to ensure seamless care

Based in Middlesbrough, we serve clients across the North East and North Yorkshire.

What You’ll Bring

We’re looking for someone with:

  • A relevant professional qualification (Social Worker, Occupational Therapist, Physiotherapist, or Nurse)
  • At least 3 years’ post-qualification experience, including work with brain injury, spinal cord injury, and/or amputation
  • Minimum 2 years’ experience in case management
  • Registration with your professional body
  • Strong knowledge of rehabilitation pathways
  • Self-employed status with professional indemnity insurance
  • A car and business insurance for travel

Pay & Checks

  • Competitive self-employed rates (negotiable based on experience)
  • Enhanced DBS required (we can arrange this if needed)
  • Proof of right to work in the UK, professional registration, references, and driving licence (minimum 12 months driving experience) required
