We are looking for a motivated and compassionate Triage Behaviour Change Coach to support individuals in making sustainable lifestyle changes that improve their health and wellbeing. You will work directly with participants across Bath and Northeast Somerset to facilitate positive behaviour change using evidence-based approaches, tailoring support to individual needs based at our Everyone Health office in Bath. This role is ideal for someone passionate about prevention, empowerment, and person‑centred care.
Key responsibilities
- Support service users with an initial triage into appropriate services provided by the Community Wellbeing Hub, providing empathetic and motivational support to ensure a professional entry into the service.
- Provide day‑to‑day triage support within Everyone Health, handling telephone calls/messages, electronic communication, appointment management and updating service user records.
- Use clinical systems to manage service user data for accurate record keeping.

Summary of Responsibilities and Duties
- Act as the main point of contact throughout the service user journey, supporting navigation and access to interventions within the service and wider B&NES community services.
- Provide day‑to‑day triage support including telephone calls/messages, electronic communication, appointment management, and updating service user records.
- Offer additional support for Everyone Health contracts when needed.
- Handle administration tasks including postal duties.
- Escalate service user issues and concerns to specialist teams when necessary, using agreed mechanisms to document need, action and result, respecting skills boundaries.
- Utilise a range of communication methods – digital technologies, telephone, email and text – to ensure patient‑led support is proportionate to individual needs.
- Be compliant with all relevant policies, procedures and guidelines, including safeguarding, and act on concerns in accordance with Everyone Health and Local Authority policies.
- Take responsibility for own CPD and participate in Everyone Health’s induction, training and continual professional development as required.
- Prioritise, organise and manage own workload to produce high‑quality work.
- Ensure equality and diversity commitments are reflected at all stages in planning, delivery and evaluation of services.
- Adhere to strict confidentiality and safeguarding protocols.
- Provide support during contact centre opening hours as required.

Qualifications Required:
- Education to GCSE standard at grades A‑C for a minimum of 5 GCSEs (or equivalent) including Mathematics and English language or literature.
