An exciting opportunity is available for a passionate, enthusiastic, dynamic and motivated Weight Management Practitioner. This role will contribute to the, provision and coordination of a Lifestyle Service delivered across Southend-on-Sea.
This community-based, multi component service brings together a range of different programmes under one Citywide Lifestyle Service, centred on supporting residents to lead healthier lives. Playing a pivotal role within this holistic, integrated service, you will be responsible for the co-ordination of a Citywide multidisciplinary team, by providing high quality planning, evaluation and strategic development for behaviour change programmes. Based across the city, this role is crucial in supporting the Service Manager as well as providing service support for members of the specialist multi-disciplinary team.
We would like to hear from you if you have the following skills and experience:
- Providing a leadership role in the planning, development and delivery of a Tier 2 weight management service
- Project co-ordination and activity planning with multiple community and professional groups.
- People management including groups, professional colleagues and local communities
- Evidence of experience in embracing and implementing change to provide high quality health outcomes.
- Understanding the key drivers for health including behaviour change theory and practice and implementing Tier 2 weight management interventions
