Work with local authorities, NHS partners and community organisations to build strong referral pathways, reduce health inequalities and increase participation among priority groups.
Oversee the exercise referral programme, develop new community-based initiatives, and support teams to deliver measurable health and wellbeing outcomes for local residents.
