Everest House Surgery is looking for an enthusiastic and patient-focused Clinical Pharmacist to join our friendly, supportive and forward-thinking multidisciplinary team.

This is an exciting opportunity to play a key role in delivering high-quality, personalised care alongside experienced GPs, nurses and other healthcare professionals.

As our Clinical Pharmacist, you will provide expert clinical pharmacy support, helping to ensure the safe, effective and evidence-based use of medicines.

You will undertake structured medication reviews, support patients with long-term conditions, respond to medication queries, complete medicines reconciliation following hospital discharge, and contribute to prescribing safety and medicines optimisation.

Working collaboratively across the practice, you will help improve patient outcomes, enhance access to care and support quality improvement initiatives, clinical governance and the continued development of our pharmacy service.

Main duties of the job

As the practice's Clinical Pharmacist, you will play a key role in supporting the safe and effective use of medicines across the practice while helping to deliver high-quality, patient-centred care.

You will undertake structured medication reviews, support patients with long-term conditions, provide expert advice on prescribing and medicines optimisation, and complete medicines reconciliation following hospital discharge.

You will also respond to medication queries from patients and clinicians, helping to ensure prescribing remains safe, evidence-based and aligned with current clinical guidance.

Working closely with GPs, nurses and the wider multidisciplinary team, you will contribute to improving patient outcomes, reducing medicines-related risks and enhancing the overall quality of care.

You will also support prescribing audits, quality improvement initiatives and the continued development of clinical pharmacy services within the practice.

  • Clinical Care
  • Undertake patient-centred Structured Medication Reviews (SMRs) and medication reviews, either face-to-face, by telephone or via remote consultation, in line with current clinical guidance.
  • Manage medicines for patients with long-term conditions, ensuring prescribing is safe, effective and evidence based.
  • Assess, prescribe and optimise medicines within your scope of practice and Independent Prescribing qualification where applicable.
  • Support the management of patients with complex medication needs, including those taking high-risk medicines or experiencing polypharmacy.
  • Provide clinical advice to patients regarding medicines, adherence, side effects and lifestyle measures to support better health outcomes.
  • Promote shared decision-making, enabling patients to make informed choices about their medicines.
  • Lead on medicines optimisation initiatives to improve prescribing quality, safety and cost-effectiveness.
  • Undertake medicines reconciliation following hospital discharge and ensure accurate prescribing records are maintained.
  • Respond to prescribing and medication queries from patients and members of the practice team.
  • Identify opportunities to improve prescribing in line with national guidance, local formularies and best practice.
  • Support repeat prescribing processes and contribute to the safe management of repeat medications.
  • Multidisciplinary Working
  • Work collaboratively with GPs, nurses and the wider multidisciplinary team to deliver integrated, patient-centred care.
  • Participate in clinical discussions relating to patients with complex needs and contribute to appropriate care planning.
  • Liaise with community pharmacies, hospital colleagues and other healthcare providers to ensure continuity of care during transitions between services.
  • Signpost or refer patients to other services where appropriate.
  • Quality Improvement & Clinical Governance
  • Contribute to clinical audits, prescribing reviews and quality improvement projects.
  • Support the practice in meeting national, contractual and quality standards relating to prescribing and medicines optimisation.
  • Maintain accurate and contemporaneous clinical records using EMIS Web, including appropriate SNOMED coding.
  • Work in accordance with practice policies, professional standards and relevant legislation, including information governance and safeguarding requirements.
  • Professional Responsibilities
  • Maintain registration with the General Pharmaceutical Council (GPh C) and practise in accordance with professional standards.
  • Maintain professional competence through continuing professional development (CPD) and mandatory training.
  • Participate in practice meetings, multidisciplinary discussions and educational activities.
  • Contribute positively to the continued development of clinical pharmacy services within Everest House Surgery.
  • Undertake any other duties appropriate to the role and level of responsibility, as agreed with the Practice Partners or Practice Management Team.

Person Specification

Qualifications

  • Master of Pharmacy (MPharm) degree (or equivalent recognised pharmacy qualification).
  • Current registration with the General Pharmaceutical Council (GPh C).
  • Successful completion of the Centre for Pharmacy Postgraduate Education (CPPE) Primary Care Pharmacy Education Pathway (PCPEP), or currently undertaking the pathway, or willing to enrol upon appointment.
  • Postgraduate Diploma in Clinical Pharmacy (or equivalent postgraduate qualification).

Experience

  • Experience managing patients with common acute and long-term conditions within the scope of pharmacy practice.
  • Knowledge of medicines optimisation, prescribing safety, medicines reconciliation and repeat prescribing processes.
  • Strong organisational and time management skills, with excellent attention to detail.
  • Excellent communication and interpersonal skills, with the ability to build effective relationships with patients and colleagues.
  • Ability to work both independently and collaboratively as part of a multidisciplinary team.
  • Proactive approach to problem-solving, service improvement and delivering high-quality patient care.
  • Commitment to patient safety, confidentiality and continuous professional development.
  • Experience working as a Clinical Pharmacist within general practice, primary care or another NHS setting.
  • Experience undertaking Structured Medication Reviews (SMRs) and medicines optimisation clinics.
  • Experience supporting prescribing audits, quality improvement initiatives or clinical governance activities.
  • Understanding of General Practice, including Quality and Outcomes Framework (QOF) and enhanced services.
  • Experience using EMIS Web or other GP clinical systems.
  • Knowledge of current NHS guidance, national prescribing standards and primary care best practice.
