Job Summary
We are looking for a creative and innovative individual to join our health facilitation team. You will work to improve access to mainstream, primary and acute services, promote health‑enhancing activities and improve outcomes for people with learning disabilities. You will provide specialist guidance and leadership within the framework of a multi‑disciplinary team, visiting acute hospital settings on a frequent basis.
Responsibilities
- Explore, develop and recommend ways of identifying and improving interventions to meet the health needs of individuals with learning disabilities.
- Lead innovation in developing effective models of care delivery, for example nurse‑led clinics.
- Clinically lead the effective delivery and timely implementation of health facilitation for people with learning disabilities.
- Develop and maintain effective links with Primary Health Care partners to support interventions, reduce health inequalities, and aid implementation of health screening and Action Plans.
- Develop and deliver training to Birmingham GP practices, hospitals and coordinate the Self Assessment Framework for LD in support of joint commissioning.
- Provide teaching, clinical presentations and representation at stakeholder meetings, linking to the wider health economy and local MDT.
- Maintain effective links with the public Health Department.
- Work in partnership with Birmingham Community Health care clinicians and Primary, Mental, and Acute services to deliver an evidence‑based approach to health facilitation.
Essential Qualifications / Training
- Registered Nurse
- First degree level or equivalent competency in a health‑related subject
- ENB 998 level or equivalent
- Evidence of continuing professional development (CPD)
Desirable Qualifications
- Leadership qualification
- Teaching qualification
Essential Experience
- Experience in community working with learning disabilities
- Experience in a health facilitation role
- Broad knowledge of access to primary health care
- Experience of supervising staff
- MDT and multi‑agency working experience
- Involvement in clinical audit and service improvement
- Experience delivering training and presentations
Essential Skills / Knowledge
- Knowledge of health facilitation
- Good interpersonal skills
- Competent problem‑solving abilities
- Knowledge of complexities within learning disabilities
- Knowledge of relevant legislation and policies
