The Clinical Hub Coordinator (Registered Nurse) role is based within the Locality Hub working as part of an integrated health and social care team to support the co‑ordination of patient care across the Locality. The Locality Hub co‑ordinates care across acute, community, and primary care settings, reducing length of stay and avoiding unnecessary hospital admissions, focusing on intervention and prevention to meet the local needs of the population within the locality.

Key Responsibilities

  • Manage the coordination of patient care within the locality, triaging referrals and messages to ensure patients are placed on the correct clinical pathway, under the supervision of the Clinical Team Leader.
  • Perform clinical hub functions including in‑reach into the aligned acute hospital.
  • Ensure delivery of safe, high‑quality care to patients across various settings.
  • Provide a holistic approach to patients/clients by conducting clinical assessment, triaging referrals, evaluating information, and establishing the correct clinical pathway based on priority.
  • Conduct locality clinical in‑reach functions in collaboration with the acute hospital, identifying patients with community‑amenable presentations who may be discharged with appropriate community support.
  • Liaise with acute, community, primary care, and social care teams to initiate ongoing referrals and integrated care pathways, supporting seamless service flow.

Working Pattern

The service operates between the hours of 8:00 am to 6:00 pm, Monday to Friday. Shifts can be arranged over 4 long days or 5 7.5‑hour shifts.
