Are you passionate about improving services for citizens? Are you self‑motivated, highly organised, and confident working with a wide range of stakeholders? At the Birmingham and Solihull Community Care Collaborative, we are looking for reliable, flexible, and proactive individuals to join our Integrated Neighbourhood Teams as Coordinators.

In this role you will work with professionals across health, social care and the voluntary sector to help deliver more joined‑up, personalised care for people within our communities. You will play a key part in improving communication, supporting meetings, coordinating actions and ensuring that information is shared effectively to achieve better outcomes for citizens.

While aligned to a specific Integrated Neighbourhood Team, you may be expected to cover colleagues when required and travel across sites for meetings and events. The Coordinator will serve as a central point of contact and navigator between the designated Primary Care Network and the multi‑disciplinary team, establishing effective communication processes across primary care, community services, mental health services and the voluntary/third sector.

The Coordinator will liaise with key workers to manage patient information, support triage and MDT meetings, capture outcomes and evaluations, and facilitate decision‑making.
