Are you an experienced Clinical Pharmacist looking for a unique opportunity? We are looking for a pharmacist with a proven track record supporting acutely unwell medical and surgical patients, to work in the Virtual Hospital in West Essex. The team cares for acutely unwell patients in homes, enabling them to remain at home by preventing unnecessary admissions and supporting early discharge. The post holder will provide specialist clinical pharmacy support to the Virtual Hospital, delivering medicines‑related care, supporting medicines management services and participating in training and education. This is an opportunity to shape services that improve healthcare experience for the local population and support an NHS national priority.
Main duties
- Provide specialist clinical pharmacy support to the Virtual Hospital MDT, accepting referrals from acute hospitals and community providers.
- Deliver expert medical treatment, clinical advice and personalised medication management plans in real time to keep acutely unwell patients at home or support early discharge.
- Forge strong professional relationships with nurses, paramedics, allied health professionals and doctors, and provide practical clinical solutions.
- Supervise junior colleagues and perform responsible pharmacist duties as required.
- Participate in the weekend and bank holiday dispensary rota, and be available for out‑of‑hours services as part of the staff bank.
- Ensure compliance with GPhC registration and demonstrate clinical credentials.
Qualifications & Expectations
- Registered with the GPhC and able to demonstrate clinical credentials.
- Proven experience supporting acutely unwell medical and surgical patients in a clinical pharmacy role.
- Excellent communication skills and a passion for clinical pharmacy.
- Flexible, willing to work out‑of‑hours and additional hours within the pharmacy team.
- Commitment to professional development and responsive teamwork.
