Job overview

We are looking for an enthusiastic, honest, self-motivated colleague who is able to provide high quality person centred care to patients, their families and carers within the Specialist Dementia and Frailty Service Multi-Disciplinary Team (MDT) in a range of settings in the West Essex Area.

Main duties of the job

The SDFS team provides a comprehensive assessment, diagnostic and treatment pathway supporting patients of any age with dementia as well as older adults over 75 years of age, who have functional mental health needs with an associated age-related frailty.

We work closely with our EPUT community specialist practitioners and Essex County Council colleagues, local organizations as well as the voluntary sector to support the holistic needs of the patient with a person centred collaborative approach.

The successful candidate will be trustworthy, positive about change and passionate about the services we provide. They will be able to work independently and autonomously, as well as part of a team, therefore will need excellent verbal and written communication skills and excellent interpersonal skills.

The successful candidate will have the opportunity to develop their knowledge and skills through working alongside our well-established MDT and accessing specialist training.

Education/Qualifications

  • Registration Mental Health Nurse RMN Part 3
  • Evidence of continual professional development
  • Meet the requirements for revalidation as set out by the NMC
  • High standard of education
  • Post graduate qualification or commitment to work towards this

Knowledge

  • Understanding of and the ability to conduct assessments and review of care in line with the requirements of the mental health care clusters
  • Understanding and the ability to support the team to meet their key performance indicators and contractual targets
  • Experience in the performance management and supervision of lower banded staff
  • Excellent IT skills (Word, Excel, PowerPoint, Outlook)
  • Experience of using an electronic information system
  • Experience of and participation in audit and research
  • Knowledge and understanding of health legislation, principles of CPA and Care Management, Mental Capacity Act, Mental Health Act 1983 and safeguarding, the NHS Constitution, and risk assessment/management
  • Knowledge of current best practice and models within nursing practice in mental health
  • The ability to devise comprehensive care plans that incorporate effective, individualised and evidenced based therapy interventions
