Overview

An Ambulance Triage Nurse (Ambulance Support) is sought to join a fast paced urgent community care pathway, providing urgent crisis response to individuals experiencing mental distress within the community. You will work jointly with the Ambulance Paramedic clinician to provide immediate crisis care across the Mid and South Essex, support other Paramedic crews in managing patients with complex mental health needs, and collaborate with non-statutory and statutory services to support people in crisis.

You will conduct initial assessments, agree a formulation of need, and develop collaborative care plans underpinned by NICE guidance, delivering a range of interventions based on individual needs. Assessments and interventions may take place in patients’ homes, team bases, hospitals, other community settings, or via tele-coaching and assisted technology and online forums. The successful candidate will work within the Mid and South Essex area conducting assessments and interventions over a 24-hour period.

Responsibilities

  • Work within the team to accompany colleagues for face-to-face support to Ambulance Paramedic crews in service users\\\' homes.
  • Provide and oversee the management and care of patients; produce accurate and timely documentation in line with professional Code of Conduct and Trust policy.
  • Conduct and manage maintenance of accurate records for all service users.
  • Collaborate with colleagues and services to support people in crisis and ensure safe, effective care pathways.
