Position Details

  • Type: Temporary (3 months)
  • Full Time
  • Pay: Up to £225 PAYE / £288 Umbrella
  • Location: Chelmsford
  • Working Style: Anywhere worker

Responsibilities

  • Provision of statutory Social Care and Social Work Services for adults aged 18+ who have been discharged from an acute or rehabilitative health provision on a pathway 1 (assessment within the adult’s home environment to determine long‑term needs after discharge from hospital).
  • Provision of Social care Occupational Therapy Services, in conjunction with the OT Hub.
  • Perform Care Act assessments outside of acute hospitals after the adult(s) have been discharged back home for within their discharge to assess period.
  • Liaise with the Safeguarding team regarding incoming safeguards and attend the PAH Safeguarding Scrutiny Panel meetings.
  • Apply a strengths‑based approach to all referrals and deliver person‑centred care, embedding a Social Model of Disability.
  • Travel around Essex (Mid covers Braintree, Chelmsford and Maldon) and maintain mobility (driving licence and car or other means as required).

Qualifications & Experience

  • Social Worker registered with Social Work England with 3 years post‑qualifying experience.
  • Broad understanding of statutory responsibilities and commitment to meeting them in a person‑centred way.
  • Experience of chairing meetings and developing strong relationships with partner agencies and the local community.
  • Ability to travel across Essex and hold a valid driving licence.
