The Role
ECC Adult Social Care continues its transformation from a focus on long‑term care support to a model built on four key principles: prevention, early intervention, enablement and safeguarding. The goal is to help people, their families and carers access information and tools to live healthily and independently, responding proactively to growing service demand while respecting the Council’s financial constraints.
You will support adults and their families with complex social needs, especially those who are more vulnerable due to disability, exclusion or diminished capacity, and maximise their potential to live independently. You will manage a caseload, including risk assessment and purposeful intervention through direct work, commissioned services and review. With a focus on the most complex and challenging adult cases, you will ensure safeguarding and share best practice with a multi‑disciplinary team as a Social Worker. This dynamic role promotes innovative person‑centred partnership working with adults, their informal carers and statutory and non‑statutory services to support mental health, well‑being and outcomes.
The Team
The ASC Integrated Neighbourhood Team assesses, reviews and safeguards adults at home or in residential placements. The team works closely with health and other partners within the neighbourhood to enable adults and their carers to live safely and independently and to prevent readmission to hospital. We are looking for a passionate Social Worker to join our Neighbourhood team. The team operates a 7‑day rota, with a required 37‑hour work week Monday‑Friday 9:00 to 5:30.
About Us

About you
You should be a qualified social worker with current Social Work England registration and at least 3 years post‑qualifying experience (including a minimum of 2 within ASC). You have significant experience modeling good practice and providing advice and guidance in complex situations. As a subject‑matter expert you possess a comprehensive understanding of current legislation, regulations and guidance. Experience chairing meetings with clients, family members, carers and multi‑agency professionals is essential. You should be able to develop strong relationships with partner agencies and the local community to support vulnerable adults.
You’ll also bring:
- Evidence of continuing professional development and capability of practice in accordance with current professional standards.
- An in-depth understanding of the financial implications of commissioned care packages.
- The ability to make clear decisions, highlight and limit potential risks associated with interventions.
