Hours: 37 hours

ECL are looking for an Acute Facilitator to join the MidEssex team based in Chelmsford. This role is to carry out and facilitate hospital discharges in line with ECL’s referral process within the Chelmsford hospital.

About You

  • Hold or to be working towards a QCF Level 3
  • Have previous experience of working within a care environment.
  • An eye for details to ensure customers' records are accurate and up to date.
  • Ability to confidentially build business relationships with health and social care partners
  • Experience of dealing with sensitive data and understanding of information governance.
  • Ability to learn and use IT systems used in the service
