The role provides dedicated analytical capacity to embed Population Health Management (PHM) across the division and the Integrated Care System (ICS) at place level.

Key responsibilities:

• Bridge public health intelligence and NHS data‑led service planning.
• Design, target, and evaluate interventions that reduce health inequalities, improve outcomes, and enhance patient experience.
• Use existing datasets to support segmentation, forecasting, and equity audits.
• Prepare the system for future linked data capabilities.
