Are you a dedicated, flexible, reliable, and enthusiastic care professional with a passion for supporting people within their own homes? If so, then a role within our Home Care Assessment and Reablement Team (HART) might be right up your street.

About the Role:

The HART service helps people to return home from hospital or prevents them from being admitted, by supporting them to regain their independence. As a HART Support Worker, you will provide short, intensive reablement support to people with disabilities and those who are frail and recovering from illness or injury, focusing on helping them to restore their optimum level of independence. You will be required to motivate them to help them build their confidence by identifying their goals and providing positive feedback about their progress and achievements. The support you provide may include helping with daily living tasks such as washing, dressing/undressing, showering, bathing, meal preparation, domestic activities, and engaging in community activities. You will also be required to support with identifying suitable assistive technology equipment and demonstrating its safe use, as well as advising on strategies for managing fatigue and pain, and preventing falls. You will be required to work shifts on a rota that operates between 7.00am and 10.00pm including evenings, weekends, and bank holidays. A DBS enhanced check for a regulated activity is required for this post.

What we offer:

  • Guaranteed contracted hours (between 15 and 30 hours per week and various geographical areas in Leicestershire).
  • Rotas are organised well in advance; with alternate weekends off work.
  • Opportunities to progress within a large public sector organisation.
  • DBS check paid for by us.
  • Excellent Local Government Pension Scheme.
  • Generous annual leave entitlement.
  • Access to staff wellbeing and counselling service.
  • Access to an employee discount scheme, providing a whole host of retail discounts including your supermarket weekly shop and gym membership!
  • Work for an organisation which 95% of our staff say is a good employer.

About You:

To apply for this post, you must:

  • Hold a Level 2/3 Diploma in Health and Social Care (or other relevant equivalent qualification) or an equivalent level of relevant experience.
  • Be committed to delivering high standards of care/support in an anti-discriminatory manner.
  • Be able to work effectively on your own as well as part of a team.
  • Have basic IT skills sufficient to write and maintain records.
  • Be able to communicate effectively both verbally and in writing and record relevant and accurate observations.
  • Be able to work under pressure and to remain calm in crisis situations.
  • Be willing to attend relevant job-related training as required.

We also expect you to share our commitment to our values and will ask you to evidence when you have demonstrated them as part of the selection process.
