About the Role

As a Community Health Reablement Worker, you will meet with individuals, families and carers to connect and listen. To deliver a community-based enablement and inclusion service, which supports people to learn or relearn skills, enable individuals to become as independent as possible, reduce social isolation, and encourage people to be part of their community. Where there is an eligible need, you will devise a plan for how the person can access the support they require, this may be connecting with their local community, referral on to other agencies or provide service to support their unmet need. Where necessary, you will identify and report potential safeguarding, consider mental capacity, and other high-risk situations where a person’s safety or liberty may be at risk. You will undertake visits to people in their own homes, in a hospital setting or wherever the person feels most comfortable to meet you.

Our Team

The Young Adult Disabilities team (YAD) is a county wide service who support young people with an Education Health and Care Plan (EHCP), who are likely to have needs for care and support as an adult to ensure those eligible social care needs will be met when they become 18. The Care Act stipulates that preparation for adulthood should begin well before a young person reaches the age of 18. It is therefore crucial that the YAD Team can begin the process of co-working and complete the adult social care assessment and support plan at an appropriate stage prior to the young person’s 18th birthday. Good preparation for adult life needs to start early and high aspirations are crucial to success. The focus should be on the young person’s strengths and capabilities and the outcomes they want to achieve. To plan successfully, all services involved in the young person’s life need to actively engage and support the planning for adulthood. An essential part of your role will be working collaboratively with children’s services, education, and health. An enhanced DBS check is required for this post.

About You

To apply for this post, you must be able to evidence in your application how you meet the following criteria:

  • Have relevant health/social care qualifications to NVQ Level 2 or equivalent and/or equivalent relevant experience/or experience of working with adults in a social care setting and of working with other agencies within a fieldwork environment.
  • Have the relevant experience (paid or voluntary) working with adults with mental health conditions, physical and learning disabilities in a social care setting and experience of working with other agencies and understand the need to undertake need and risk assessments and the issues/needs that adults and their families/carers may have.
  • Knowledge of the needs and issues of citizens and carers including ethnic minorities, disability and age issues and an understanding of disability issues and how this can impact on people’s lives.
  • Understanding of recovery, enablement, social inclusion and Person-Centred approaches.
  • Have a good understanding of social care organisations and their roles and responsibilities as part of the wider community and voluntary sector.
  • Be a confident communicator who is skilled in negotiation, persuasion, assertiveness, and conflict management, and able to build effective working relationships.
  • Be able to recognise the limits of own accountability and responsibility, and to seek appropriate support and advice when required and be able to manage a caseload and work to clear objectives and have the ability to work/travel to various locations across the county, work as part of a duty rota for the service and the ability to work flexibly.
