The Transfer of Care Hub has undergone a large amount of changes over the last 18 months in order to adapt to changing demands and improve the discharge process.
We are passionate about ensuring the people of North East Essex are supported to maximise their health opportunities and promote a home first ethos within the trust.
We are looking for a Trusted Assessor to join our developing team working within the acute and community hospitals providing a seven-day service. You will be responsible for developing a closer working relationship between care providers and the acute hospital. The role will sit within the Transfer of Care Hub but will be working on behalf of care providers assessing and ensuring patients are relevant for services in line with the discharge to asses model.
You will be required to complete detailed care assessments and should have knowledge of the Health and Social Care Act and NHS Continuing Healthcare process. You should have a clear understanding of the implications and requirements of discharge for care providers & what they are able to provide.
We are looking for staff who are organised, have experience of effective multi-agency working & developing relationships between services.
Demonstrate a high level of expertise with regard to improving process and speed up the discharge of patients to or back to a care home
Provide care homes with assurance around the impartiality of the assessor and assessment.
Facilitate the discharge of patients into a care homes to avoid unnecessary delays and ensure patients go home as quickly as possible
Reduce mis-communication between the hospital and Care Home and build trust
Reduce the need for Care Home practitioners needing to come into the hospital to assess, freeing up time and resources
Help support the discharge planning process with the wards and care homes
