The West Essex Hospital at Home Service within EPUT is seeking an experienced Band 6 Clinical Practitioner to join our multidisciplinary team. You will assess, plan and deliver care at patients’ homes, with remote monitoring support, and work flexibly across seven days to avoid hospital admission where possible.
You will be registered nurse or allied health professional with post‑registration experience, capable of autonomous decision‑making and clear communication.
