This is a great opportunity to join a dynamic Primary Care Network in Epsom, Surrey. The role offers a mix of home visits with close liaison with our practices and our community team (matrons, DNs, physiotherapists etc.) and a practice-based role within a large, thriving practice in the centre of Epsom.

The Paramedic will work autonomously both within the community and within the PCN and Practice based multi-disciplinary team.

Main duties of the job:
  • Assess patients presenting with acute illness or for review of chronic disease conditions.
  • Undertake home visits for acute illness or chronic disease management.
  • Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs and follow up as appropriate.
  • Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services.
  • Perform specialist health checks and reviews; perform and interpret ECGs; perform investigatory procedures as required.
  • Undertake the collection of pathological specimens including intravenous blood samples, swabs etc.
  • Support the delivery of anticipatory care plans as this process develops (e.g. monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing).
  • Provide an alternative model to urgent and same day GP home visit.

Epsom PCN brings together six GP practices across the Epsom and Ewell area, serving a population of approximately 63,000 patients. We are a well-established and forward-thinking network, supported by a multidisciplinary team of around 25 to 30 additional roles, including Clinical Pharmacists, Paramedics, Physician Associates, First Contact Physiotherapists, Pharmacy Technicians, Care Co-ordinators, GPs, and a dedicated Health & Wellbeing Team.

We work closely with our Community and Integrated Mental Health Teams to provide joined-up, person-centred care. Epsom PCN is proud to be among the first 43 areas in England selected for the government's new Neighbourhood Health Programme, reflecting our strong partnerships and collaborative working with the VCSE (Voluntary, Community and Social Enterprise) sector.

Our PCN is dynamic and ambitious, demonstrated by our success in securing two Surrey Heartlands test bed pilot projects including one focused on delivering transformative care at scale through an innovative new Electronic Patient Record (EPR) system. It's an exciting time to join Epsom PCN as we continue to shape the future of integrated care in our community.

Job responsibilities:

The Paramedic/Practitioner will work autonomously both within the community and within the PCN and Practice based multi-disciplinary team, using their clinical assessment and treatment skills, to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems.

Core Duties:
  • Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
  • Diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
  • Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.
Other Duties:
  • Work within the policies of scheme of the practices and PCN.
  • Promote client involvement in the management of the service.
  • Participate in regular supervision sessions and appraisals with your line manager.
  • Attend training and development activities as identified and participate in meetings as required.
  • Maintain administration systems and workspace used in a clean and tidy condition.
  • Work flexibly to meet the needs of clients.
  • To undertake any other duties appropriate to the grade and purpose of the job as may be agreed by the post holder.
Communication:

Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment. Communicate effectively with patients and carers recognizing the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.

Health & Safety:

Maintain a good working knowledge of Health and Safety procedures and fire precautions, and operate the correct procedures and participate in policy development and data collection where appropriate.

Confidentiality & Data Security:

All employees are expected to comply with all Practice policies and procedures related to confidentiality and data security and to work in accordance with General Data Protection Regulations.

Teaching:

Assist in the teaching of GP Registrars, medical students, nurses, HCAs and any other staff in training.

Professional:

Adhere to the Royal College of Paramedics Code of Professional conduct, scope of professional practice and maintain HCPC registration.

Person Specification:
  • Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans.
  • Ability to write comprehensive clinical notes, implement and evaluate care plans.
  • Working towards advanced practitioner status.
  • Experience of advanced care planning.
  • Experience of training/supervision.
Qualifications:
  • BSc in a training programme approved by the College of Paramedics.
  • Full UK driving licence.
  • Pre-reg MSc in a training programme approved by the College of Paramedics.
  • Evidence of verification against the paramedic primary care roadmap.
