We have an exciting opportunity to recruit a Band 7 Heart Failure Specialist Nurse for Sutton's Heart Failure – Cardiology Team.

Responsibilities

  • Manage a caseload of patients with heart failure, including those newly diagnosed and following discharge from hospital.
  • Liaise with hospital consultants, GPs and other members of the heart failure team along the cardiac pathway from hospital to home, delivering evidence-based care to patients with heart failure.
  • Work autonomously while maintaining close contact with partners in primary, secondary and social care as well as voluntary agencies.
  • Co-ordinate an education programme for community health professionals and act as a resource for advice.
  • Work collaboratively within multi-disciplinary teams and in partnership with other services to provide seamless care.
  • Contribute to the team’s success by being an effective role model and supervising other staff (e.g., nurses, healthcare assistants/support workers and students) to ensure they work to the standards the people using our services deserve.
  • Support flexible working arrangements and discuss any flexibility needed during the recruitment process.

Qualifications and Experience

  • Experienced nurse who enjoys a challenge and has good communication skills.
  • Excellent clinical skills and the ability to work with a high level of autonomy.
