The Reablement Assistant will function as part of the SH&C Reablement Unit providing care, support and rehabilitation for patients within the Unit and within their own home. The post holder will undertake a range of delegated tasks, follow care plans and report daily to the Nurse in charge and Therapy leads. The role will be supported to develop skills and abilities to work with this client group to provide the appropriate level of reablement.
It is anticipated that this role will continue to develop through the acquisition of further skills, knowledge and competencies to be determined within the clinical teams with focus on patients needs. Please refer to the job description for full details on:
