Surrey Downs Health and Care Diabetes Team have an exciting opportunity for a Band 3 Healthcare Assistant to join our community team across Surrey Downs. It involves assisting to deliver effective care and supporting our patients with their Diabetes care providing expertise, educating patients and clinical staff with regards to Diabetes Technology.

The Diabetes Community Team see patients within Clinic settings across 7 PCN’s. We offer excellent opportunities for training and development of both your skills. The team has very experienced Clinical Lead who will ensure that you have the necessary support for your personal development. The community Diabetes service works over a 5-day week 8.30am to 16.30pm Monday to Friday

This post requires an interest in working within a community setting.

You will be supported with a comprehensive induction and training as needed, to help you settle in, with the time and opportunity to get to know the people on our caseload, along with development opportunities.

Main duties of the job

Role Summary

To support the Diabetes Specialist Team in providing patient education.

To ensure patients are receiving appropriate health screening and checks related to their diabetes management and promptly communicate any concerns to the Diabetes team.

To be a contact for patients receiving care from SDHC Specialist Community Diabetes service.

Detailed job description and main responsibilities

Principle Duties to include:

COMMUNICATION AND ENGAGEMENT

  • To provide support to the patients to help them make informed decisions about their care. This support can be offered through face-to-face, telephone discussions, or by responding to email queries.
  • To provide individualised diabetes care tailored to patient needs and circumstances.
  • To provide diabetes education to patients, covering topics such as hypoglycaemia management, insulin initiation teaching and to completion of the Gold score questionnaire.
  • To ensure smooth running of the clinics by supporting clinicians in various duties, including blood pressure monitoring, weight measurement and blood glucose or ketone monitoring.
  • To support patients to download their glucometer data onto Diasend before their appointment time.
  • To liaise with patients regarding annual health checks to improve patient safety
  • To support diabetes team with health promotion and awareness events such as Insulin Safety Week, Hypo Awareness Week and World Diabetes Day. This will includes sourcing promotional materials, organising venues and stands, advertising the events and engaging with public.
  • To provide education to patients on how to use blood glucose and ketone meter.
  • To ensure education materials are accessible to patients. This may require liaising with company representatives for leaflets, study materials and demo kits.
  • To support the diabetes team in delivering Freestyle Libre and Bertie educational sessions.
  • To ensure diabetes stocks are up to date and to order any items that have run out from representatives.
  • To maintain folders for use in diabetes clinics containing relevant educational materials.
  • To establish and maintain effective communication with diabetes team and patients.
  • To maintain a professional attitude and appropriate appearance at all times in accordance with Trust Dress Code Policies.
  • To improve patient experience and satisfaction in diabetes management.
  • Foster effective communication channels between the diabetes team and patients, addressing inquiries and concerns promptly and professionally.
  • Possess a valid driving license and the ability to safely drive if required for travel between Trust sites or other job-related duties.

Person specification

Qualifications

  • Able to demonstrate literacy and numeracy skills
  • NVQ Health and Social Care level 3 or equivalent

Experience

  • Previous experience as a Health Care Assistant in an acute or community setting
  • Experience in working as part of a diabetes team
  • Quality Improvement

  • Must be able to demonstrate, in relation to patients, visitors, colleagues and other individuals connected with the Trust and NHS:
  • Effective communicate to ensure high standards of care, treatment, service or support as appropriate to your role,
  • Patient facing roles -be able to understand an individual’s communication, physical and emotional needs; recognise their needs and preferences
  • Hold valid UK driving license
  • Ability to work as part of a team, providing support to colleagues and suggesting improvements to the role.
