We have an exciting opportunity for a Band 6 Nurse to work at ICP PCN within Surrey Downs Health & Care. Responsibilities
- The post holder is responsible for the continuing management of patients and/or staff within their clinical setting, working independently alongside their team lead and assuming the chain of command in their absence.
- Responsibilities include assessment of care needs, development, implementation and evaluation of programmes of care, setting and maintaining standards, and supervising and deploying staff when appropriate.
- The post holder will facilitate educational programmes for basic, post‑basic and in‑service training in close liaison with the Learning and Development Team.
- Work closely with senior team members to facilitate patient‑focused, innovative, research‑based practice in line with Surrey Downs Health and Care objectives.
Main duties of the job
We are looking for nursing staff from all backgrounds, whether more experienced, looking for a change, wanting to work locally, or excited by the difference of working out in the community with its unique benefits and challenges. These roles require the post holder to hold a current and valid driving licence enabling them to drive in the UK and to have access to a vehicle for work purposes.
PCN context
PCN – Primary Care Network – is a network formed from GP practice populations working in partnership with others to provide health and care to local communities.
What will PCNs do?
- Provide ongoing care and support
- Deploy the most appropriate staff member(s) in a coordinated and timely way to best use community resources
- Reduce risk of admission for patients
- Support people in their homes, people in care homes and through in‑reach into hospitals where necessary
- Work closely with the community hospitals and specialist teams
Each PCN has a community‑focused trimurvate (group of three) of Lead GP/Community, Lead Clinician and Operational Manager. Specialist services and community hospitals work closely with the PCNs to ensure a whole‑system approach to working across Surrey Downs.
Learning and Development
We have our own Learning & Development Department that offers a range of different courses and development plans that give you the opportunity to grow your career.
Qualifications
- Registered Adult Nurse with the Nursing and Midwifery Council (NMC)
- Valid driving licence (to drive within the UK)
- Teaching qualification
- Evidence of continued professional development post registration
Experience
- Role model to others in both clinical and team cohesiveness
- Evidence of experience in team management and multi‑disciplinary working
