Overview

The Hepatology Surveillance Nurse, Band 6, will collaborate closely with the nurse manager and the Band 8 nursing team to establish and implement a nurse‑led liver surveillance pathway. This pathway will support patients with stable, compensated liver cirrhosis who are at risk of developing Hepatocellular carcinoma (HCC) across various outpatient settings in Epsom and St Helier NHS Trust.

Responsibilities

  • Conduct patient assessments.
  • Order investigations such as blood tests and Ultrasound (USS) scans.
  • Review blood and ultrasound results.
  • Provide lifestyle advice and signposting.
  • Perform fibroscan.
  • Deliver the HCC surveillance pathway.
  • Work closely with the Gastroenterology/Hepatology nursing team to coordinate care.
  • Conduct nurse‑led clinics for patients with stable, compensated cirrhosis.
  • Refer patients with advanced liver disease to senior Band 7/8 nurses, consultant clinics and nurse‑led fibroscan clinic.
  • Request and coordinate six‑monthly diagnostics for patients with compensated cirrhosis.
  • Track and monitor patients throughout the pathway.
  • Provide advice, information and support for participants enrolled in the liver HCC surveillance pathways.
  • Be competent to autonomously support patients to make informed decisions prior to undergoing a surveillance USS and communicate test results.
  • Implement surveillance and follow‑up in accordance with relevant society and National guidance (e.g., NICE QS152).
  • Provide specialist knowledge and advanced communication skills with patients and carers when obtaining consent for procedures and providing psychological support where needed.
  • Act as a patient advocate supporting them to navigate and make informed choices throughout the process.
  • Ensure timely access to USS surveillance for those individuals.
  • Ensure effective links with Gastroenterology/Hepatology MDT to ensure timely presentation and transfer of patients to the MDT.
  • Populate and maintain the liver surveillance spreadsheet/database.
  • Accurately use Microsoft Office, NHS net, local databases and remote connectivity to practice effectively and independently.
