Job overview

The team offers medical assessment, daily review, MDT discussion, and management of conditions.
The core function of this model is the Home First UCR/virtual ward which provides rapid assessment and wrap around care to people in their own home (usual place of residence) who become suddenly unwell and would normally be admitted to hospital or we can support with an earlier discharge from hospital.
