Team Overview
The Surrey Downs Health and Care Community Therapy Team works across organisational boundaries within the Primary Care Networks, and is responsible for providing therapeutic/MDT intervention for those who would otherwise need emergency hospital admission, as well for those medically stable to return home from an acute hospital but requiring enhanced short term support as one stage in their ongoing rehabilitation pathway. The Surrey Downs Health and Care Community Therapy Service provides innovative rehabilitation for adults, offering joined up, coordinated care to local citizens with health and rehabilitation needs. The service aims to support people living as independently as possible by offering multidisciplinary assessment and further rehabilitation. The Primary care network provides a multidisciplinary approach to support the individual and facilitates a holistic assessment within the framework of the MDT and treatment response, with the purpose of reducing risk admission to the acute system and providing individualized, targeted rehabilitation.
