We are seeking enthusiastic Community Nurses to join our staff bank for ad-hoc work across community sites based in Leatherhead, Dorking, Banstead, Epsom and East Elmbridge, including twilight services (08.00–23.20). Opportunities also exist to work in our two community hospitals (Dorking and Molesey) and the integrated 24/7 unit at Epsom Hospital. Full-time (37.5 per week) is preferred, with a schedule that may include some weekends (approximately 1 in 4). Flexibility and the ability to work across the integrated locality team within Sutton is important. Applicants must have their own car with the correct insurance.

Community Nurses deliver holistic, patient-centred care in patients’ homes and residential homes as part of a multidisciplinary team, linking closely with GP practices and other community services. The role aims to prevent unnecessary hospital admissions and help people manage their conditions at home.

Main Duties of the Job

  • Full-time work with some weekends (estimate 1 in 4)
  • Provide primary nursing care to patients in homes, residential homes, local community sites, community hospitals, and the integrated unit at Epsom Hospital.
  • Deliver care as part of a multi-disciplinary team, collaborating with GP practices and other local services.

Clinical Competencies

  • Venepuncture
  • End-of-life care, including medicines – injectable/syringe drivers
  • IVABS (not essential)
  • Wound care – compression (not essential)
  • Blood pressure, pulse, saturations – identifying an unwell patient
  • Wound dressings – ulcers & accidental wounds
  • Pressure area management and knowledge
  • PIC lines – flush and management
  • PEG – meds and flush (not essential)
  • Bowel care – suppositories, enema, DRE
  • Insulin administration
  • Injections
  • Catheters – wash-out, insertion, removal (male/female/Spubic)
  • Bladder scans
  • Dopplers (not essential)

Act as an autonomous practitioner, working with the support of senior team members. Treat all patients as individuals, respecting privacy and dignity at all times. Assess individual care needs, implement and evaluate holistic programmes of care. Inform, support and educate family/carers. Promote health and wellbeing of the patient. Develop effective communication and liaison systems. Maintain, develop and monitor standards of evidence-based nursing care. Be responsible and maintain timely patient care. Co-operate and partner with all other professionals and agencies involved in the care of the patient. Act upon procedures to protect vulnerable individuals. Maintain professional and clinical integrity in line with NMC guidelines. Ensure continuity of care between all clinical settings through appropriate timely planning and communication. Maintain clear, comprehensive, signed and contemporaneous records according to Surrey Downs Health and Care procedures. Keep knowledge up-to-date in all aspects of clinical practice, acting as a resource and guiding other staff. Ensure policies, procedures and standards of care are adhered to at all times. Support the team lead where appropriate and assist with delegated tasks and duties. Liaise with safeguarding team and social services, supporting other team members in safeguarding processes.

Additional Information

For new entrants into the NHS, pay under the Agenda for Change or Medical & Dental Pay Bands will normally be set at the minimum of the advertised pay band.
