Manage and develop the Virtual Fracture Clinic Service for the Trust. Ensure the orthopaedic trauma patients are treated appropriately in a timely manner. Manage the band 6 physiotherapist as part of their rotation from MSK department. Liaise closely with the Orthopaedic Consultant of the Week (CoW) discussing treatment plans for patients that have presented through ED and require Orthopaedic input. Manage the plan for these patients. Request bloods, x-rays, CT/MRI scanning of patient, with outcome and plan of care. Explain clearly the treatment plan with the patient for their condition. This may include advising patients of distressing news. Delegate to the junior nurse as necessary. Create and analyse reports for the VFC. Make service improvements where necessary. Input and update all patient data on database. Review waiting list numbers against waiting times. Manage improvements that can reduce unnecessary waits. Refer patients to other specialities. Evaluate service regularly and implement service improvements. To provide and maintain high standards of clinical care within the VFC and trauma specialty and ensure that the delivery of patient care reflects feedback from patients and families and National Standards and Guidelines. Lead in the audit of evidence based outcomes.

To independently undertake highly specialised clinical assessments, diagnose, treat and discharge adults and children with a wide range of minor injuries, including management of complex cases within professional boundaries and in line with their training, skills and competencies. To ensure patients and parents/carers are fully involved in the design and implementation of patient treatment plans. To ensure effective coordination and liaison with multidisciplinary teams across Primary and Secondary Care Services, statutory and voluntary disciplines/groups in order to provide a seamless service to the patients and their carers and prevent hospital admission where urgent care is required. To identify actual or potential clinical risks and take appropriate action using, when required, the appropriate reporting structure. Attend VFC meetings. Documenting full and accurate records of assessments, investigations, procedures, treatments and aftercare. Work with Orthopaedic Consultants and junior doctors to co-ordinate the delivery of trauma care. To identify and case manage patients high risk of fractures. Act as a clinical leader to ensure the assessment of individual patient care needs and the development, implementation and evaluation of plans of care are in place. Recognise and respond appropriately to urgent and emergency situations, considering patients' individual and often complex situations.

The Trauma & Orthopaedic Department at Epsom and St Helier Hospital are looking to recruit an APP to lead our VFC department. This is a maternity cover post for 14 months and will commence from June 2026. The post is for 30 hours per week, to be worked over 4 -5 days, but must include working a full day on a Monday. The post holder should have extensive experience at a band 7 or higher, and would preferably have experience in the acute setting within a Trauma & Orthopaedic service. The post holder will be the Lead for the Virtual Fracture Clinic Service. You will ensure that the Trust offers a comprehensive and cohesive service for all Orthopaedic Trauma patients. The post holder will autonomously undertake assessment and treatment of adults and children who present with an injury in the Virtual Fracture Clinic. This is to determine a clinical diagnosis, treatment plan and appropriate discharge within their professional boundaries, in line with training, skills and competencies. The post holder will lead on the management of proximal humeral fractures, Achilles ruptures and soft tissue injuries. The post holder will be integral with the Clinical Lead for Trauma in the development of a quality and innovative orthopaedic trauma service and assist in the management of the trauma service.
