Job Overview
The Community Neuro Services provides services to patients with Stroke and other long‑term neurological conditions. An early supported discharge (ESD) pathway delivers flexible therapy to ease early discharge from acute settings for new stroke diagnoses. The long‑term pathway offers a specialised multidisciplinary service to safeguard independence, prevent inappropriate hospital stays, maximise rehabilitation, and minimise premature dependence on long‑term care.
Therapists assess, differentially diagnose, plan and implement rehab interventions and manage a caseload of adults with neurological conditions in the community setting. They also educate carers, supervise students and mentor junior staff, working flexibly across the Neuro service.
Main Duties
Clinical Responsibilities
- Provide specialist neurological intervention to patients registered with GPs within Sutton CCG.
- Deliver rehabilitation to patients with neurology diagnoses meeting Community Neuro Services criteria, including those in residential or nursing homes.
- Carry out clinical assessments, treatment planning and goal‑oriented therapy interventions in community settings.
- Manage a caseload within a multidisciplinary team, ensuring clients and carers receive optimal therapy within available resources.
- Perform multi‑disciplinary screening assessments and discuss outcomes with team members.
- Keep accurate and up‑to‑date records.
- Abide by RCOT standards, NHS policies, and local health & safety regulations.
Duties & Responsibilities (Detailed)
- Participate in individual performance reviews, in‑service programmes and maintain clinical and managerial skills through supervision, CPD, and a personal development plan.
- Deputise for the Band 7 in their absence and assist the Service Manager with day‑to‑day operations.
- Maintain confidentiality and comply with the Data Protection Act.
- Adhere to Trust policies on equal opportunities, substance misuse, and no smoking.
- Implement and monitor Trust and departmental/local policies.
- Maintain professional image of the Trust and Sutton Health & Care.
Equipment Competence (EEG & Evoked Potentials)
Adult EEG:
- Perform routine calibration and maintenance; take clear clinical history; deliver routine adult and sleep‑deprived EEGs.
Paediatric EEG:
- Perform routine calibration/maintenance; take clinical history; deliver routine paediatric and sleep‑deprived EEGs; experience with children with special needs, challenging behaviour and autism.
Evoked Potentials:
- Perform routine calibration/maintenance; deliver spinal and somatosensory evoked potentials (SSEPs); perform lower limb NCS.
Person Specification
- Professional registration with a UK body (e.g., RCOT).
- Minimum 6 months post‑registration experience in occupational therapy.
- Strong clinical skill in community rehabilitation and long‑term management of disability.
- Experience in neuro assessment, differential diagnosis, treatment planning, and inter‑professional working.
- Demonstrated ability to educate and supervise.
