Job Overview: Provide rehabilitation support services to patients in the community, supporting physiotherapy, occupational therapy or speech and language therapy programmes.
Main duties of the job:
- Deliver rehabilitation support in various settings, including the department, patients’ homes or other appropriate locations.
- Manage an individual patient caseload, assessing and treating routine patients within guidelines agreed by the treating therapist.
- Monitor patient progress, make minor adjustments to treatment programmes, and report progress and any problems to the treating therapist. Act independently while receiving guidance and advice almost daily.
- Play an active role in the local healthcare economy as the lead provider in innovative health and care partnerships.
Detailed job description and main responsibilities:
- Gather information prior to seeing a client and provide feedback to the therapist.
- Obtain biographical information and knowledge from relevant sources such as medical notes and computer records. Take a simple case history. Explore other external information relating to patients.
- Contribute to the assessment process and provide feedback to the therapist.
- Carry out mobility and transfer assessments for specific pieces of equipment requested by therapy or nursing staff and report back to the qualified member of the team regarding the need for further input.
- Implement therapy programmes/approaches, both on an individual basis and for groups, and provide feedback to the therapist.
- Carry out programme devised by the therapist using a range of delegated exercises and manual physical therapy techniques.
- Work jointly with clinicians in complex cases that may require physical moving and handling as part of rehabilitation.
- Plan patient treatment programmes for approval of the therapist and implement them.
- Monitor progress and implement changes as appropriate while keeping the therapist informed.
- Undertake support work alongside the therapist for highly dependent patients with complex needs.
- Assume responsibility for assessing and managing clinical risk in relation to your own caseload, seeking guidance and help from the therapist as appropriate.
- Will not attempt any procedure without appropriate training and competency to carry out specific instructions.
- Contribute to the evaluation of therapeutic interventions, including discharge planning.
- Comment objectively on observed therapy intervention, using an outcome measurement for a specific goal.
- Adapt the treatment programme according to the outcome measurements.
- Prioritise workload, following managerial supervision.
- Prioritise and plan day‑to‑day workload.
- Identify factors involved in prioritisation and contribute to discussions about long‑term prioritisation of work.
