An opportunity has arisen for a Band 5 Occupational Therapist to join the At Home Team in Sutton Health and Care. We are looking for candidates with the relevant knowledge and skills, and enthusiasm to deliver and develop the OT workforce within the Multidisciplinary team.
We are looking for enthusiastic Occupational therapists to work as part of the Sutton Health and Care at Home service. You will be part of the MDT team that helps to prevent admission from community, accelerate discharge from emergency department and acute medical ward, Acute Frailty ward and provide supported discharge through Discharge to assess model of care through to community based Rehabilitation all within one team. We are innovative and people focused with a 'one team' ethos, which means better quality and more joined up care for our patients and families. The team strives to constantly improve service delivery and contributions from all staff are highly valued. We will provide professional development opportunities and support you to provide great care to our patients.
Main Duties
- Deliver skilled occupational therapy services across the team.
- Have clinical responsibility for assessment and treatment planning for people referred.
- Participate in developing and monitoring the service.
- Work closely with multidisciplinary teams and other professionals, particularly in relation to patient enablement, and assist in developing good working relationships with statutory, voluntary and independent organisations and agencies.
- Actively participate in audit, service progression and role development.
Qualifications and Experience
- Degree in Occupational Therapy.
- HCPC registration.
- Experience of acute working.
Personal Attributes
- Ability to work as part of a team.
- Ability to prioritise work.
Skills and Knowledge
- Good written and spoken communication skills.
- Experience of carrying out audit.
