Overview

An exciting opportunity at SHC which provides therapy services within Epsom and St Helier Hospital and the community in Sutton. SHC – At Home service works closely with Acute services at St Helier to provide a detailed service to mainly Sutton and Merton residents with rehabilitation needs. This post is currently under Sutton Health and Care, mainly to cover community service, with opportunities to work across various pathways within the team including elderly rehabilitation, Acute Medical Unit, A&E/front door, Acute Frailty ward, and home-based rehabilitation. The post holder must participate in rostered weekend work within the team.

Responsibilities

  • Provide a high standard physiotherapy service within the service.
  • Perform physiotherapeutic assessment of patients with diverse presentations of physical and psychological conditions, provide a diagnosis, and develop and deliver an individualised treatment programme.
  • Take responsibility for own caseload and a defined area of the service, or a particular patient type, working with and without direct supervision. Supervision includes regular formal training, clinical reasoning sessions, peer review and case conferences.
  • Access advice and support from a senior physiotherapist as required; clinical work should be routinely evaluated.
  • Undertake all aspects of clinical duties as an autonomous practitioner.
  • Liaise effectively with other disciplines.
  • Undertake evidence-based audit and research projects to improve own and the team\\\’s clinical practice within each rotation; make recommendations to the clinical lead/manager for changes to practice and may assist with implementing changes or contributing to service protocols.
  • Undertake training/study to ensure competency and emergency duties as a lone practitioner within the Trust.
