Job Description

Qualified Social Worker, Manchester Council, Adults Hospital Discharge Team – Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team.

The team works extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager.

The responsibilities will include:

1. Support hospital discharges from acute hospital settings and non-acute NHS community settings.
2. Complete assessments in a timely manner to ensure residents are discharged from hospital safely.
3. Adult Placement.
4. Report writing.
5. Home Visits.
6. Liaise with other professionals.
7. 36 hours per week (flexible by negotiation).
8. 2 Month Temporary Contract.
9. Start date subject to DBS and reference clearance.

The successful candidate will need to hold a DipSW or equivalent, must be registered with the HCPC, have a current DBS, and be a car driver.
