Job Overview

This post is based within a local authority integrated health and social care service that brings together many disciplines.
The Care Coordinator role sits within the social work team and will focus on face‑to‑face work with service users, families and external partners.
Responsibilities
- Conduct face‑to‑face work with service users, families and external partners.
- Identify whether support packages continue to meet the needs of service users and their families.
- Make recommendations regarding support packages and arrange/amend support to ensure needs are being met in a person‑centred, strength‑based way.
- Participate in the integrated front‑door duty service on a rota basis, supporting the resolution of crisis situations.
- Undertake assessment work and support safeguarding case work.
- Attend multidisciplinary meetings and present cases.
- Communicate effectively with service users, families and a range of internal and external colleagues.
- Write accurate, comprehensive support plans and assessments.
- Required Skills
Excellent verbal and written communication skills. Ability to work within multidisciplinary teams and make person‑centred case recommendations.
