The LPN Program Manager is responsible for the clinical oversight and daily operational management of our residential group homes. You will bridge the gap between medical necessity and quality of life, ensuring that our residents receive high-quality nursing care while maintaining a warm, home-like environment. You will lead a team of Direct Support Professionals (DSPs) and LPNs, ensuring compliance with state regulations and individual service plans.

Key Responsibilities

  • Clinical Oversight & Care Coordination
    • Health Monitoring: Oversee the medical needs of residents, including medication administration, wound care, and chronic disease management.
    • Assessment: Conduct regular health assessments and update Individualized Service Plans (ISPs) to reflect changing medical needs.
    • Triage: Act as the primary clinical point of contact for emergencies and coordinate with physicians, pharmacies, and hospitals.
  • Staff Leadership & Training
    • Supervision: Hire, train, and supervise LPNs and DSPs, ensuring they are competent in delegated nursing tasks.
    • Education: Provide ongoing training on Medication Administration (Med Tech certification), infection control, and specific health protocols.
    • Scheduling: Manage nursing and care rotations to ensure 24/7 coverage and regulatory compliance.
  • Compliance & Quality Assurance
    • Documentation: Audit Medication Administration Records (MARs) and nursing notes to ensure 100% accuracy and regulatory alignment.
    • Regulatory Liaison: Serve as the lead during state audits and inspections (e.g., CMS, DHS, or local health departments).
    • Safety: Lead the Risk Management committee to analyze incidents and implement preventative measures.

Required Qualifications

  • Licensure: Active, unencumbered Licensed Practical Nurse (LPN) license in this state.
  • Experience: Minimum of 3–5 years of nursing experience, preferably in IDD (Intellectual and Developmental Disabilities), Geriatrics, or Assisted Living.
  • Leadership: At least 1 year of supervisory or management experience.
  • Clearances: Ability to pass a comprehensive background check and drug screening.
