Job Summary
Imperial College Healthcare NHS Trust\’s strategy seeks to deliver our vision of ‘better health, for life’. It is rooted in a set of core values – to be kind, aspirational, collaborative and expert – and is focused around three strategic goals:
- help create a high quality integrated care system with the population of north west London
- develop a sustainable portfolio of outstanding services
- build learning, improvement and innovation into everything we do
The work of the strategy, research, innovation & improvement directorate, led by our Executive Director Prof Bob Klaber, primarily focuses on the delivery of our third strategic goal – ‘to build learning, improvement and innovation into everything we do’. The directorate includes sub‑functions of improvement, innovation, strategy & partnerships, sustainability, population health & equity (to which these roles directly relate) and life sciences.
We are therefore seeking two passionate and engaging ‘Population Health Senior Project Managers’ to deliver an ambitious set of objectives to improve population health and health equity.
Main duties of the job
- Positively influence the health of north west London by embedding a population health approach across all services and finding ways to better serve communities outside the hospital walls, focusing on health equity, prevention and our role as an anchor organisation.
- Coordinate and deliver population health projects and improvement initiatives in response to identified needs.
- Work closely with clinical, operational and community partners to design, implement and evaluate interventions that improve healthcare access, experience and outcomes for populations with the greatest health needs.
- Provide a greater focus on projects in collaboration with clinical services at the Trust (one role) and on engaging with local communities and voluntary organisations (second role). Both roles will collaborate on health equity and prevention projects.
- Both are fixed‑term (24‑month) roles with at least 50% in‑person working required at our main hospital sites. We support flexible working but require a minimum 0.8 WTE.
